Diabetes

The CHER Diabetes Education Center is a community-based outpatient education center committed to providing up-to-the-minute information to people with diabetes. Accredited by the American Diabetes Association, there are locations throughout the city.

Staffed by Registered Nurses, Registered Dietitians and Certified Diabetes Educators, the Center offers the following services to both groups and individuals:

  • Diabetes self management education
  • Blood glucose monitoring
  • Flexible meal planning
  • Physical activity
  • Medications
  • Behavioral changes
  • Insulin management
  • Insulin starts
  • Insulin pump therapy and instruction
  • Insulin pump upgrade
  • Continuous glucose monitoring
  • Pre-diabetes (referral not required)
  • Gestational diabetes
  • Medical nutrition therapy

Our education services are targeted toward people with Type I, Type 2 and gestational diabetes. Whether you are newly diagnosed or have been living with diabetes for years, our classes and educators will provide the information you need to begin a workable management plan. Consultations and presentations are also available through Northwest TeleHealth video conferencing. Some services require a physician referral.
